Hyer Function Membership & Program Terms

1. Program Overview

Hyer Function provides physician-led telemedicine programs focused on metabolic health, hormone optimization, and medically guided weight management.

Programs may include:

  • Physician medical evaluation and follow-up

  • Ongoing medical oversight

  • Wellness coaching support

  • Nutrition consultation with a Registered Dietitian

  • Laboratory evaluation and monitoring

  • Educational resources and lifestyle guidance

Specific services included in each program will be described at the time of enrollment.

2. Membership Structure

Hyer Function programs operate on a membership model.

Membership may include:

  • Initial medical consultation

  • Ongoing physician follow-up

  • access to wellness coaching and nutrition support (depending on tier)

  • secure messaging or clinical support through approved platforms.

Membership fees cover clinical services and program participation and do not guarantee prescriptions or specific treatments.

3. Initial Consultation Fee

An initial consultation fee is required prior to beginning services.

This visit includes:

  • comprehensive medical history review

  • evaluation by a licensed physician

  • discussion of treatment options

  • development of a personalized care plan if clinically appropriate.

The consultation fee is non-refundable once the visit has occurred.

4. Monthly Membership Fees

After the initial consultation, participation in Hyer Function programs requires a monthly membership fee.

Membership fees cover ongoing care coordination and clinical oversight.

Fees may include:

  • physician follow-up visits

  • care coordination

  • wellness coaching sessions

  • nutrition consultations (depending on program tier)

Membership fees do not include the cost of medications, laboratory testing, or pharmacy charges unless specifically stated.

5. Minimum Program Commitment

Certain Hyer Function programs require a minimum program commitment to allow sufficient time for safe medical monitoring and treatment evaluation.

For many programs, this may include a minimum four-month commitment.

If applicable, this requirement will be disclosed at enrollment.

6. Provider Clinical Discretion

All medical decisions are made solely at the discretion of the treating provider.

Participation in a Hyer Function program does not guarantee:

  • eligibility for treatment

  • prescription medications

  • hormone therapy

  • weight loss medications

  • laboratory testing.

Providers may determine that certain treatments are not medically appropriate.

7. Prescription Medications

Prescriptions may be issued only after a medical evaluation by a licensed healthcare provider.

Certain medications used in Hyer Function programs may include:

  • hormone therapy medications

  • weight management medications

  • metabolic support therapies.

Medication availability may vary based on:

  • medical eligibility

  • clinical judgment

  • pharmacy supply

  • regulatory requirements.

8. Laboratory Testing

Laboratory testing may be required before or during treatment to ensure safe and appropriate care.

Patients are responsible for:

  • completing recommended laboratory tests

  • associated laboratory fees if not included in the program.

Failure to complete recommended labs may result in delayed treatment or discontinuation of services.

9. Compounded Medications

In some situations, a provider may determine that a compounded medication is appropriate.

Compounded medications:

  • are prepared by licensed compounding pharmacies

  • may not be FDA-approved

  • may be used when commercially available options are not appropriate or available.

Patients will be informed if compounded medications are prescribed.

 

10. Patient Responsibilities

Patients participating in Hyer Function programs agree to:

  • provide accurate medical information

  • follow medical guidance provided by their healthcare provider

  • complete recommended laboratory testing

  • attend scheduled follow-up appointments

  • communicate changes in health status or medications.

Failure to comply with medical recommendations may impact treatment eligibility.

11. Membership Cancellation

Patients may cancel their membership in accordance with program terms.

Cancellation requests must typically be submitted prior to the next billing cycle.

Membership fees already charged for the current billing period are generally non-refundable.

 

12. Practice Termination of Services

Hyer Function reserves the right to discontinue services if:

  • a patient provides inaccurate or misleading information

  • a patient fails to follow medical recommendations

  • abusive or inappropriate behavior occurs

  • a provider determines treatment is no longer medically appropriate.

Where appropriate, reasonable notice and referral options may be provided.

13. Telemedicine Acknowledgment

By enrolling in Hyer Function programs, patients acknowledge that certain services may be delivered through telemedicine technologies.

Patients understand that telemedicine has limitations and that in-person care may be recommended when clinically appropriate.

 

14. Limitation of Liability

Hyer Function provides medical services consistent with accepted medical standards.

However, health outcomes cannot be guaranteed.

To the extent permitted by law, Hyer Function is not liable for damages resulting from misuse of medical advice or failure to follow medical guidance.

15. Governing Law

This Agreement shall be governed by the laws of the State of New York.

16. Acknowledgment

By enrolling in Hyer Function services, you acknowledge that:

  • you have read and understood these Membership Terms

  • you agree to the policies described in this document

  • you understand that medical decisions are made at the discretion of your healthcare provider.
